SMOOTHING PATCH TEST FORM

At Billi Currie, we recommend a patch test at least 48 hours prior to receiving the Aura Smoothing Treatment to check for any possible sensitivity or allergic reaction to the products used. 

NB: A reaction is very rare. 

I confirm that I have been advised that a patch test is recommended before undergoing this treatment.

I understand that smoothing treatments contain active ingredients that may cause reactions in some individuals. These reactions, although rare, may include: Redness, Itching, Skin irritation, Sensitivity to treatment vapours

I understand that a reaction may still occur even if I have previously had smoothing, keratin, or similar treatments without any issues.

BILLI CURRIE HAIR SALON MODEL AGREEMENT

This Billi Currie Hair Salon Model Agreement ("Agreement") is entered into between [Model Name], located at [Model Address], and Billi Currie Hair Salon, located at [Salon Address], effective as of [Effective Date].

SERVICES: Billi Currie Hair Salon agrees to provide the following services to the Model:

  • Haircut

  • Hair styling

  • Hair colouring

  • Other hair treatments as agreed upon by both parties

APPOINTMENTS: The Model shall schedule appointments in advance for desired services. Billi Currie Hair Salon agrees to make reasonable efforts to accommodate the Model's preferred appointment times.

LIABILITY: Billi Currie Hair Salon shall not be liable for any damages, including but not limited to, allergic reactions, injuries, or dissatisfaction with the results of the services provided. The Model acknowledges that results may vary depending on factors such as hair type, condition, and individual preferences.

MODEL RESPONSIBILITIES: The Model agrees to disclose any relevant medical conditions, allergies, or sensitivities that may affect the services provided. The Model also agrees to follow any post-service care instructions provided by Billi Currie Hair Salon to maintain the results of the services.

INTELLECTUAL PROPERTY: Model acknowledges that any designs, styles, or techniques used by Billi Currie Hair Salon are proprietary and may not be reproduced without the Salon's consent.

ENTIRE AGREEMENT: This Agreement constitutes the entire understanding between the parties concerning the subject matter hereof and supersedes all prior agreements and understandings, whether oral or written.

IN WITNESS WHEREOF, the parties here to have executed this Agreement as of the date first above written.
